Pediatric Care
At DCHC, our Child, Youth, and Family Clinic (CYFC) brings together several different health care professionals to help meet the needs of pediatric clients throughout Durham region. The CYFC consists of a primary care Nurse Practitioner, Psychotherapists, Lactation Consultant, Registered Nurse, and Medical Secretary who all work together to provide exceptional care specifically for clients from birth to 18 years of age.
Goals:
- Improve timely access to primary care, counselling and pediatric services
- Facilitate referrals to specialized services
- Support clients in navigating referrals to other health and social supports with DCHC and partner agencies/providers
Services:
- Newborn screening and assessment
- Well-baby/Well childcare
- Developmental and growth assessment/monitoring
- Breastfeeding support
- Preventative health care
- Immunizations (children and adults)
- Treating acute (sudden onset) or episodic illness
- Referrals to/coordination with other health care professionals
- Chronic Illness management
- Health Education
Eligibility:
- Children from birth to 18 years of age and new moms
- Do not have existing primary care provider
- Resident of Durham Region
Patient/client referrals will be assessed and then the patient will be contacted to book an appointment with the appropriate provider(s).

- Completion of the Children, Youth and Family Clinic Referral form
- Fax completed form to 905-720-1292 or drop off at our North Oshawa location (1320 Airport Blvd., Oshawa ON)
