HART Hub
The Durham Community Health Centre (DCHC) Homelessness and Addiction Recovery Treatment (HART) Hub provides low barrier services to support individuals experiencing homelessness and addiction in the Region of Durham. The HART Hub offers comprehensive wraparound supports to help clients meet their health and social needs.
The DCHC HART Hub aims to:
- Increase affordable housing options (supportive housing)
- Improve collaboration/coordination across local organizations
- Increase access to mental health and addictions services
- Provide culturally competent services and equitable care
DCHC HART Hub Partners
The DCHC HART Hub is a collaboration that includes the following key partners:
- Durham Community Health Centre
- Regional Municipality of Durham
- GraceWins
- Victim Services of Durham Region
- Lakeridge Health
The DCHC HART Hub also works closely with other health and social service partners in the community to support HART Hub clients.




Who We Serve
The DCHC HART Hub provides outreach and services to individuals in Durham Region who are:
- Currently on Durham Region’s By-Name List*
*The Durham Region By-Name List: Individuals who are experiencing homelessness and who have provided consent to receive support and be matched with appropriate housing resources.
Programs and Services
The DCHC HART Hub implementation will be taking a multi-phased approach.
Phase 1: Focuses on outreach and coordinating essential services including intensive case management, peer support, primary care, and social services (e.g., income, housing, life skills, employment supports) to engage and support clients.
Phase 2: Services will expand to include mobile clinics, health promotion, mental health and addiction supports, and supportive housing, delivered through decentralized, community-based outreach services.
Phase 3: When fully operational, all services will be integrated and centralized, providing comprehensive care outreach, health services, social supports, mental health and addictions support, and supportive housing (e.g., housing retention, stability, re-integration). More details will be shared as the implementation progresses.
