HART Hub Whitby
The Homelessness, Addiction, Recovery Treatment (HART) Hub Whitby provides welcoming, wrap‑around care for adults in Durham Region who are experiencing homelessness, housing instability, mental health challenges, and/or substance use concerns.
As part of Ontario’s Homelessness and Addiction Recovery Treatment (HART) initiative, the Whitby HART Hub is designed to support people who may find it difficult to access the right services when they need them most. Many individuals facing homelessness and complex health needs encounter barriers to care. The HART Hub brings essential supports together in one place to make care easier, more coordinated, and more responsive.
What We Offer
At HART Hub Whitby, clients can access integrated, person-centered services that may include:
- Primary health care
- Mental health and substance use supports
- Harm reduction services
- Case management and navigation
- Connections to housing and community resources
Our team works alongside each client to understand their unique goals and provide compassionate, low‑barrier care. The HART Hub is committed to helping people improve health, build stability, and access the resources they need, without judgement and without the requirement for appointments or referrals.
Who Can Access The HART Hub
HART Hub Whitby welcomes adults (18+) who are experiencing:
- Homelessness or housing instability for 6 months or longer
- Multiple health challenges
- Substance use concerns including individuals seeking coordinated supports to make changes to their substance use
- Barriers to accessing health and social services
No referral or health card, or appointment required.
Emergency and Crisis Services
Whitby HART Hub does not provide emergency response or emergency access to shelters or crisis services. For emergencies, please call 911 immediately.
What Programs and Services are Currently Offered
Through coordinated partnerships, the HART Hub provides:
- System navigation and case management
- Connection to low-barrier community services
- Access to primary care
- Mental health support and addiction referrals
- Transitional shelter and housing navigation
How the HART Hub Works
The HART Hub connects individuals to coordinated care through a structured and supportive pathway:
- Referral received
- Intake and eligibility assessment
- Assignment to System Navigator
- Stabilization and care coordination
- Transition to housing and ongoing supports
Each client is supported by a dedicated System Navigator who coordinates services across healthcare, housing, and community partners.
The HART Hub is designed to:
- Reduce emergency department use
- Improve housing stability
- Enhance continuity of care
- Support long-term health outcomes
