Welcome to Durham Community Health Centre’s Online Appointment Booking platform!

We are pleased to offer our clients the convenience of booking appointments online. This platform is only available for existing clients. New clients must contact our office directly to set up their first appointment. You can book an appointment if you have:

• An active OHIP Number, or
• Your Chart ID

When booking your appointment online please select the option that applies to you (see options below). This ensures you are directed to the correct booking page.

Before You Book:

To ensure a smooth booking process, please:

• Have your OHIP number or Chart ID ready
• If you do not have a chart ID, contact the office to receive one before booking.
• Confirm you are booking only with your provider.

Please note:

• Annual check-ups cannot be booked online. If you need an annual visit, please contact the office
• Appointment types and availability are limited. If you do not see a suitable appointment option, please contact the office.
• Book appointments only with your provider. Appointments made with another provider will be cancelled.
• Appointments may be adjusted or cancelled by our office if the appointment type does not match your needs, requires more time, or provider availability.
