DCHC provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few.

To refer your client, please browse the options below and select the team or pathway that best suits the needs of your client. Please note, our programs are non-exclusive, meaning clients receiving support from one team can also receive support from another.

The Gender Care team consists of Nurse Practitioners, Registered Nurse, Systems Navigator, Peer Support Facilitator, Therapist
Our goal is to provide gender transition related care and services within a holistic framework for a time-limited length of service
Services Provided:
- social, legal and medical transition support
- hormone related therapy
- surgery referral support, advocacy
- health system navigation.
- We also offer support, education, and resources for families and caregivers of our clients.
For Primary Care Providers: Our team offers telephone consultations for PCPs interested in learning how to start and maintain people on hormone therapy. Click here for more details.
How to Refer your Client:
Clients can call the Clinic Coordinator to schedule an appointment 289-509-0603 ext. 3203.
Eligibility:
- Children and youth aged 3-19 years and their families
- Resident of Durham Region
Services Provided:
- Single session counselling – offered in person or virtually
- Intake appointments takes place on Tuesdays and appointments are scheduled on Wednesdays

Eligibility Criteria: anyone of any age, that identify as Black, African, or Caribbean.
Goals: To providing culturally-affirming care to help clients feel understood, validated, and supported in their cultural identities and empower them to explore and understand how their cultural backgrounds play a role in their overall health and well-being. While offering specialized programming tailored to the needs of ACB (African, Caribbean, and Black) community members.

Eligibility Criteria: Anyone of any age, that identifies as indigenous, and would like the cultural supports.
Goal: To support health and well-being of Indigenous community members through culturally appropriate care, activities, and community connection. It focuses on providing inclusive programs (events, workshops, and gatherings) that promote wellness, cultural identity and sense of belonging.
How to Refer your Client: DCHC IPC Mental Health Referral Form – completed by physician/nurse practitioner and faxed to DCHC.
Team: Mental Health Nurse Practitioner & Therapist
Goal: To strengthen the integration of primary care services and improve access to care in Durham Region for individuals aged 14 and up who are struggling with mood, anxiety, and/or with/without symptoms of psychosis
Eligibility criteria:
- Age Requirement: Clients must be at least 14 years old.
- Referral Source: Clients without access to allied health services are referred by primary care providers (external and internal), urgent care centres, or community agencies within the Durham region.
- Location: Clients must reside in the Durham Region.
- Mental Health Conditions: Conditions such as depression, anxiety, and PTSD, among others, that can benefit from an interprofessional team approach.
- Social-Economic Barriers: Characteristics like lack of an OHIP card, financial assistance, housing, social support, etc., which hinder access to care.
- Complex Health Needs: Conditions requiring integration of medical management and social support services.
Exclusion criteria:
- Immediate safety concerns for the client
- Actively suicidal or homicidal, or having a clear plan
- Severe anxiety and agitation
- Acute psychosis or uncontrolled bizarre behaviour
- Concern for the client’s welfare
- Acutely intoxicated or severe alcohol and/or substance use.
How to Refer Your Client:
Client must self-refer by calling: 1-877-227-3217.
Eligibility criteria:
- Location: Clients must reside in the Durham Region.
- Social-Economic Barriers: Low income and who do not have other means to pay for counselling, such as coverage through employee benefits plans, school/educational settings, or the ability to pay for private therapy OR those who experience barriers to care based on their identity or circumstances, including gender, sexual orientation, or physical ability, etc.
- Current Service use: Not receiving counselling elsewhere
- Engagement and readiness: Attending voluntarily and willing to work on specific goals
- Service expectations: Comfortable with counselling services that are brief in nature (6-12 sessions)
Exclusion criteria:
- Are experiencing severe mental health concerns or active psychosis
- Require long-term or intensive services
- Are currently in crisis

Team: Hepatologist, Treatment Nurse, Outreach Worker, Case Manager and Program Coordinator
Services Provided:
- Hepatitis C education, testing, and treatment
- Psychosocial reports
- Treatment options
- Bridging at-risk groups to clinical, community services and advocacy
- Peer support and Hepatitis C support groups
- Harm reduction education, supplies and prevention, as well as Naloxone.

Team: Registered Nurses, Registered Dietitians, Coordinator, consulting endocrinologist
Eligibility criteria:
At DCHC, the Diabetes Team provides education to those who are:
- Residents of Durham Region
- 18 years or older
- At risk of or diagnosed with Prediabetes or Type 2 Diabetes

Team: Nurse Practitioner, Occupational Therapist, Pharmacist, Behavioural Supports Clinicians, Home & Community Care Service Coordinators
Goal: Provide specialized geriatric care to support frail seniors (generally 65 years plus) living at home or in retirement residences, who have multiple complex medical and social problems including:
- Cognitive impairment
- Responsive behaviours
- Decreased function
- Falls or risk of falls
- Impaired mobility
- Incontinence and/or
- Multiple medications
