Eligibility

  • Durham Region residents
  • seeking long-term primary care supports
  • not already connected with a primary care provider (or are looking to switch primary care providers).

 

Disclaimer: While our Mobile Clinic provides essential primary care services, it is not equipped to handle emergencies such as crises, overdoses, heart attacks, respiratory distress, or other life-threatening conditions.

Led by Nurse Practitioners, the Primary Care Mobile Clinic (PCMC) team aims to attach individuals to a primary care provider, and offer long-term support, wrap-around services, and culturally affirming and holistic care. By working closely with various community partners, the Mobile Clinic team aspires to improve health equity, awareness, and access to clinical, wellness and health education services within Durham Region.

With a predominant focus towards newcomers, uninsured, and unattached individuals, DCHC’s Primary Care Mobile Clinic aims to remove barriers for priority populations by working in collaboration with local organizations to offer multiple clinic access points throughout the community.
*Please note, you do not need to identify as a newcomer to access this service.*
