Pediatric Care
At DCHC, our Pediatric Care Team brings together several different health care professionals to help meet the needs of pediatric clients throughout Durham region. The Pediatric Care team consists of a primary care Nurse Practitioner, Psychotherapists, consulting Pediatrician and Medical Secretary who all work together to provide exceptional care specifically for clients from birth to 18 years of age.
The integrated clinical and wellness care provided by our Pediatric Care Team focuses on improving timely access to primary care, counselling, and pediatric services. Our Pediatric Care Team goals are to ensure that timely and consistent medical services are provided by relevant care providers and to help facilitate prompt referrals to specialized services. The professionals within the Pediatric Care Team can also support clients in navigating referrals to other health and social supports within DCHC programs and partner agencies/providers.
Any patient or client experiencing physical or psychological disorders and needing inter-professional services within Durham Region can be referred to the Children, Youth & Family Clinic. These referrals can come from multiple sources such as, primary care providers, DCAS, urgent care centres and other community programs.
Patient/client referrals will be assessed and then the patient will be contacted to book an appointment with the appropriate provider(s).
How to Refer
Referrals can be implemented through this Children, Youth & Family Clinic Referral Form and via self-referral.
