Our 2026–2031 Strategic Plan

With guidance from our Board of Directors, Durham Community Health Centre created a new Strategic Plan with input from our community. We heard from staff, clients, leaders, and partners through interviews, focus groups, and workshops.

This plan reflects what matters most: care that is easy to access, culturally safe, and well coordinated. It also highlights the importance of working together and learning from both data and real-life experiences. As our communities grow and change, we are adapting how we deliver care.

Our 2026–2031 Strategic Plan focuses on improving care, supporting our staff, and building strong partnerships to better serve Durham Region. Guided by compassion, trust, and collaboration, this plan shows our commitment to building healthier communities together.
